What a direct deposit form is for

A direct deposit authorization form gives your employer permission to send your pay straight to your bank account, and tells payroll exactly where to send it. It’s also used for other recurring payments, such as pensions or contractor payouts. Once it’s processed, your pay arrives on payday with no check to deposit.

How to fill out a direct deposit form

  1. Choose whether this is a new authorization, a change of account, or a cancellation.
  2. Add your name, address, contact details, and employee ID if your employer uses one.
  3. Enter your bank’s name, the nine-digit routing number, and your account number.
  4. Pick checking or savings, and whether to deposit your entire net pay, a fixed amount, or a percentage.
  5. To split your pay, add a second account with a fixed amount or percentage. The rest goes to the first account.
  6. Print the form, sign and date it, attach a voided check if your employer asks, and hand it to payroll or HR.

Where to find your routing and account numbers

On a paper check, the routing number is the nine digits at the bottom left, followed by your account number. Without checks, look under account details in your bank’s app or online banking. The form above checks that a routing number has a valid format, but it can’t confirm it belongs to your bank, so always copy it from your own account.
