What goes on a direct deposit form
Every employer’s form looks a little different, but they all ask for the same details. A pre-filled form from your bank covers the bank details for you. You add the rest.
- Your name and address
- As they appear on the bank account.
- Bank name
- The bank that holds the account. App-based accounts often list a partner bank instead of the app’s name.
- Routing number
- The nine-digit number that identifies the bank. Copy it from the form, a check, or your account details, not from a search result.
- Account number
- Your checking or savings account number.
- Account type
- Checking or savings.
- Amount
- Your whole paycheck, a percentage, or a fixed dollar amount, if your employer allows a split.
- Signature and date
- Your authorization for the employer to deposit into the account.
Some employers also ask for a voided check. If you do not have paper checks, a bank-generated form that shows your account and routing numbers usually works instead. Ask payroll what they accept.
Keep your direct deposit safe
A direct deposit form contains everything needed to send money to your account, so share it only with your employer’s payroll or HR team, through the channel they normally use.
Be wary of emails or texts asking you to “update” your direct deposit details, even if they look like they come from HR. Payroll diversion scams use fake requests to redirect paychecks. Confirm any change in person or through your employer’s official payroll portal.
Use your Chime deposits as proof of income
Once your paycheck lands in Chime, those recurring deposits are a record of what you earn. Landlords and lenders often ask for recent statements. Our guide to getting a Chime statement shows where to download them.
If you would rather not send every transaction, connect your Chime account and create an income report from your bank deposits. It shows your recurring deposits and an estimated monthly income, straight from the bank.
Chime direct deposit FAQ
- What bank name do I put for Chime?
- Use the bank name on your completed direct deposit form. It is either Stride Bank, N.A. or The Bancorp Bank, N.A., depending on your account.
- Does the Chime form include a voided check?
- Chime’s completed direct deposit form PDF includes a voided check for employers that ask for one.
- How long does Chime direct deposit take to start?
- It depends on your employer’s payroll schedule. A new direct deposit often takes one to two pay cycles to start, so keep an eye on your first paycheck and pay stub.

- Yes. Once your pay lands in your Chime account, the recurring deposits show up on your statements, and you can connect the account to an income report that summarizes them for a landlord or lender.
