What goes on a direct deposit form
Every employer’s form looks a little different, but they all ask for the same details. A pre-filled form from your bank covers the bank details for you. You add the rest.

- The bank that holds the account. App-based accounts often list a partner bank instead of the app’s name.
- Routing number
- The nine-digit number that identifies the bank. Copy it from the form, a check, or your account details, not from a search result.
- Account number
- Your checking or savings account number.

- Checking or savings.
- Amount
- Your whole paycheck, a percentage, or a fixed dollar amount, if your employer allows a split.
- Signature and date
- Your authorization for the employer to deposit into the account.
Some employers also ask for a voided check. If you do not have paper checks, a bank-generated form that shows your account and routing numbers usually works instead. Ask payroll what they accept.
Keep your direct deposit safe
A direct deposit form contains everything needed to send money to your account, so share it only with your employer’s payroll or HR team, through the channel they normally use.
Be wary of emails or texts asking you to “update” your direct deposit details, even if they look like they come from HR. Payroll diversion scams use fake requests to redirect paychecks. Confirm any change in person or through your employer’s official payroll portal.
