How to fill out and serve the notice

  • Name every adult tenant. Adding “and All Other Occupants” covers anyone else living there.
  • Use the exact rental address, including the unit number.
  • On a 7-day notice, list only rent owed, not late fees or other charges, unless the lease defines them as rent.
  • Serve it by one of the methods on the form: in person, with a household member of suitable age, by first-class mail, or electronically if the tenant agreed in writing. If you mail it, allow extra days before counting the notice period.
  • Sign the notice and complete the certificate of service. Keep a copy; you'll need it to file in court.
